<p>Efficiency and quality are the hallmarks of Lean Six Sigma. But the Coronavirus, and our ability to prepare and even respond effectively to its devastating consequences, has exposed what can happen when there has been a singular focus on efficiency. The question we need to ask is this: Have we gone too far in one direction and not far enough in another?</p>

<p>Consider that Lean processes emphasize the minimization of delays, errors and waste. As a result, under pressure to emphasize efficiency and cut unnecessary waste, hospitals, in the past, have taken steps to limit inventory by maintaining stock levels to what would be needed in the short run, no more and no less. They have stocked no extra swabs, masks and gowns. Indeed, stockpiling or maintaining buffer stocks was discouraged, often considered unnecessary, or even wasteful.</p>

<p>Even the number of ventilators were limited to no more than what would be needed in normal times. And this focus on efficiency extended to human resources. The number of nurses and physicians was kept to just the right number, no more and no less. As one administrator said, “we were rewarded for trimming fat, so we trimmed fat.”</p>

<p>But the consequence of managing a Lean organization is that there is little cushion, no extra masks, swabs or gowns or even extra healthcare professionals. So, when the Coronavirus placed heavy demands on the system, healthcare professionals had no choice but to reuse single-use masks or to wear garbage bags to protect them from respiratory droplets.</p>

<p><img decoding="async" class="size-full wp-image-3799 alignright" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2020/04/patriots-plane.jpg" alt="" width="195" height="146">Shortages were so severe, and risks to healthcare workers so high, that heroic efforts were needed to locate supplies. The owners of the New England Patriots sent the team’s customized Boeing 767 to Shenzhen, China to load over one million masks and fly them back to Boston. It was a plan that required cooperation from many levels in the United States and China. Permission was finally granted under the condition that the crew remain on board the aircraft when it landed in China and that the plane remain on the ground no more than 3 hours. The obstacles were overcome, and the plane departed. Once landed, the aircraft was loaded and cleared for takeoff in 2 hours and 57 minutes.</p>

<p>Why were these shortages so severe? Why were healthcare organizations so ill prepared? Here is a quote from the Worldwide Threat Assessment Report issued by Daniel Coates, Director of National Intelligence, and submitted to the Senate Select Committee on Intelligence. The date on that report was January 29, 2019.,</p>

<p><em> <img decoding="async" class="size-full wp-image-3801 alignright" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2020/04/threatassesment.jpg" alt="" width="149" height="192">We assess that the United States and the world will remain vulnerable to the next flu pandemic or largescale outbreak of a contagious disease that could lead to massive rates of death and disability, severely affect the world economy, strain international resources, and increase calls on the United States for support. </em></p>

<p>Apparently, the warning went unheeded because less than one year after this report was submitted to Congress, the coronavirus spread from Wuhan, China to the United States. From the beginning of that outbreak, hospitals and public healthcare professionals warned that they were unprepared for the devastation the pandemic could wreck and the shortages that would prevent proper treatment for those who fell ill.</p>

<p>As we all know, cases increased at an exponential rate, and deaths mounted. One estimate was that as many as 200,000 people would die in the United States. And just one month after the first outbreak, the number of cases soared in the United States with over 25,000 cases diagnosed in New York and 200,000 across the entire country. It was already late to stem the onslaught of the disease.</p>

<p>Toward the end of March, all attention turned to how the pandemic could be contained and mitigated. In addition to social distancing, and the closures of non-essential businesses and events, one very critical step was to test patients. But the healthcare system was unprepared. It needed more test kits, more swabs, more reagents, more masks, and more gowns. And without test data, it would make containment and mitigation very difficult.</p>

<p><img loading="lazy" decoding="async" class="size-full wp-image-3800 alignright" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2020/04/test-patients.jpg" alt="" width="213" height="134">To test patients, processes needed to be set into place. Not just any process, but efficient processes; processes that minimized delays, errors and waste. Following the lead from South Korea, drive-through sites were established, many in parking lots adjacent to hospitals. It was a strategy to protect both patients and healthcare workers and get the job done efficiently. Then, processes were designed to speed test results. Originally, it took days to receive results, but with improvements in testing technology, the delay was reduced to minutes.</p>

<p><img loading="lazy" decoding="async" class="size-full wp-image-3802 alignright" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2020/04/ventalators.jpg" alt="" width="207" height="127">It also became apparent that hospitals were at risk of running out of ventilators. With about 60,000 ventilators across the country – not including 100,000 older versions - the projected need was over 500,000 units, some estimates as high as 960,000. What followed was a significant effort to encourage manufacturers, such as the Ford Motor Company, to meet this shortfall by developing manufacturing processes capable of delivering ventilators as quickly as possible.</p>

<p>Even the discovery of a therapeutic drug or the development of a vaccine was under intense time- pressure. They needed to be made available as soon as possible. And the scientific community responded. Barriers were broken as scientists from all over the world shared data and research directed toward a single goal. What normally would take years to develop, test and move through trials was expected to take one-and-a-half years or less.</p>

<p>While the late response contributed to untold hardship, and most importantly to the thousands of lives that had been lost, there are lessons here that must inform future efforts as we must prepare for the likelihood of future pandemics expressed in the World Threat and Assessment report.</p>

<p>At the Hospital level, two critical processes must be evaluated. First, the supplies that support diagnosis and support must be adequate to meet emergencies. Where appropriate adequate safety or buffer stocks of equipment must be maintained.</p>

<p>Second, the processes that support the diagnosis and care for patients must also be reviewed. Are there steps that can be taken to prepare for the rare but devasting events that can overwhelm our healthcare facilities? How do we accommodate an unprecedented increase in patients? How can this be done efficiently?</p>

<p>Third, at the Federal and State level, we need guidance from such agencies as the Centers for Disease Control and the National Institute of Health to develop strategic plans that balance short term needs - that demand one type of efficiency - with long-term needs - that demand very different types of efficiency. This guidance can help to better understand the risks and costs that need to be taken into consideration. Further, this guidance can help to develop strategies for acquiring, mobilizing and sharing resources during such emergencies.</p>

A few of our competitors have unleashed malicious attacks directed at our company. And these attacks have followed several very successful years in which our programs have gained widespread acceptance and praise from professionals, organizations and universities across the world.</p>
<p>While there is no basis for these attacks, what is clear is that the objective of these offenders is to create doubt in those who are interested in SSGI’s offerings and divert them from SSGI’s site to their own site.</p>
<p>Here is the revealing part, these attacks are not just directed at us, but also at almost all of our major competitors in the online Lean Six Sigma space. In a very systematic fashion, the intention of these offenders is to narrow the choice for training to their own sites.</p>
<p>We have no intention of naming these offenders, because that is not how we prefer to compete. Unfortunately for them, they only succeed at damaging their own reputation, and raising concern about their own credibility. Do not, however, take our word for it. We strongly recommend that you do your own research on SSGI. We are confident that if you go to Google and compare SSGI to the offending organizations, the differences will speak for themselves.</p>

Yet, we are still responsible for delivering customer value, a concept at the center of every Lean Six Sigma initiative.</p>
<p>Yet, how do we keep up with these changes? How do we ensure customer value? The answer is simple. We must establish and maintain an organizational culture that is committed to continuous improvement.</p>
<p><img loading="lazy" decoding="async" class="size-full wp-image-2574 alignright" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2019/07/warrenbennis.jpg" alt="continuous improvment six sigma" width="177" height="178" srcset="https://www.6sigmacertificationonline.com/wp-content/uploads/2019/07/warrenbennis.jpg 177w, https://www.6sigmacertificationonline.com/wp-content/uploads/2019/07/warrenbennis-150x150.jpg 150w" sizes="(max-width: 177px) 100vw, 177px" /><a href="https://en.wikipedia.org/wiki/Warren_Bennis" target="_blank" rel="noopener">Warren G. Bennis</a>, professor of business at the University of Southern California and a pioneer in the field of leadership said it very well, “In life, change is inevitable. In business, change is vital.” To keep our organizations vital, then, continuous change is an absolute necessity.</p>
<p>The improvements brought on by a culture of continuous improvement do not necessarily have to be major or disruptive. Rather, it is even more important to create a culture where many of these changes are small, incremental and continuous, and where these changes focus on continually improving the design, processing and delivery of the organization’s products and services.</p>
<p><img loading="lazy" decoding="async" class="size-full wp-image-2575 alignleft" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2019/07/youngernext-year-book.jpg" alt="" width="75" height="122" />There is an interesting book written by a Crowley and Lodge, “Younger Next Year.” It captures the concept of continuous improvement and its basic premise is that normal aging is not normal and that through a consistent program of exercise people can become more lean and fit as they age. When followed, the authors promise better and better health every year. But the authors warn that most people can’t go from where they are today to where they want to be instantly. If they expect that outcome, they will be disappointed. Instead it is more reasonable to expect slow and continuous improvement.</p>
<p><img loading="lazy" decoding="async" class="size-full wp-image-2572 alignright" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2019/07/continuous-improvment-lean-six-sigma.jpg" alt="continuous improvment lean six sigma" width="156" height="131" />Continuous improvement applies across the organization but has become increasing important in product and software design. One expression of this concept is called Minimum Viable Product or MVP. It is an approach to design where a simple but functional product is introduced early into the marketplace. Then, based on feedback, the product is subjected to a series of improvements through time. In software, new releases, offer more and more functionality and, because they embody improvements, they frequently command better and better market share.</p>
<p>However, continuous improvement is far more difficult than it sounds. Why? Resistance to change.</p>
<p>People don’t like change. It’s human nature. But they do respond in a crisis. In in a Harvard Business Review article by John Kottler titled “Leading Change: Why Transformation Efforts Fail”, he suggests that resistance to change can only be overcome during an organization crisis. Underscoring the importance of change, Schaffer in a more recent Harvard Business Review article contends that all management is change management. This certainly explains why a good part of management’s responsibilities must overcome resistance to change.</p>
<p>Another source of resistance can be traced to financial pressures. Often improvement requires abandoning some approaches while introducing others; equipment must be scrapped, old procedures abandoned and even personnel changed. This can be costly.</p>
<p><img loading="lazy" decoding="async" class="size-full wp-image-2571 alignleft" src="https://www.6sigmacertificationonline.com/wp-content/uploads/2019/07/Boeing-Six-Sigma.jpg" alt="Boeing Six Sigma" width="207" height="139" />The <a href="https://www.6sigmacertificationonline.com/cost-of-poor-quality-boeing-max-8/" target="_blank" rel="noopener">Boeing 737 Max 8</a> was one of the most successful aircraft in aviation history. When two of them crashed and it became obvious that designers needed to return to the drawing board and “improve” their design, management resisted grounding the remaining planes. At stake were financial repercussions in the billions of dollars. In this situation, safety trumped financial pressures and the entire fleet of planes was grounded for months.</p>

<p>While many companies have a formal recruiting process, too often those involved in the process are so busy with other responsibilities that the length of time that a prospective new hire remains in the recruiting process is far too long. And the longer this process stretches the greater is the likelihood that the candidate is no longer available.</p>
<h2>Take a Close Look at Process Flow. Is it Lean?</h2>

<h2>Process Delays Are Costly</h2>
<p>Delays commonly occur between every one of these steps. Some of them extending for days or weeks. When they are long, the best candidates are likely to be lost to competitive offers, and the company forced to consider applicants further down the list. At worst, the process is started all over again.</p>

<p>Indeed, many companies suffer from inefficient hiring processes, but in periods when the job market is soft there are likely to be many good candidates from which to choose. So, inefficient processes in that situation have few consequences. But in tight labor markets, or when the kinds of skills that are in demand are scarce the consequences are significant. In these tight markets every competitive organization needs to eliminate process delays.</p>
<h2>Quick Action to Hire the Right Person</h2>
<p>There is one problem, says, Shore, “When re-engineering the hiring process, it must be lean enough to assure that the right person is hired, but not so lean that the wrong person is hastily hired. The challenge is to strike the balance.”</p>
<p>Nonetheless, for most companies there are many opportunities to improve process efficiency and eliminate delays, errors and waste. Together these problems prevent efficient hiring practices. But in addition to process efficiency, these processes must be monitored and controlled to assure that efficiency is maintained.</p>

<p>According to Dr. Barry Shore from the University of New Hampshire and Six Sigma Global Institute, two lean six sigma tools stand out. They include Value Stream Maps and Control Charts. Here is why they are so fundamental to almost every project, why they contribute so much to a successful outcome, and why they are so essential to project success.</p>
<h2><strong>1. Value Stream Maps</strong></h2>

<p>Every process, whether it delivers products or services, can be visualized as a flow of physical units or people from the beginning to the end of the process. And before we can fully understand these processes, it helps to create a clear picture of what steps are involved. Only through a clear picture can the ambiguities that often describe a process be clarified. Only through a clear picture can there be an understanding among stakeholders of how the process really works and what needs to be done to improve it.</p>

<p>That clear picture can be captured in a Value Stream Map. It identifies a logical sequence of steps, including delays, that define the process. It is a map that anyone can read, from Len Six Sigma professionals to top management.</p>
<p>In fact, several Value Stream maps are frequently necessary, one for the current state and another for the proposed state.</p>
<p>Since clarity is so essential in achieving a positive project outcome, no project should avoid this step.</p>

<p>If a process is worth the time and effort to improve it, then the development and use of control charts is worth the time and effort to ensure that outcome metrics endure over time.</p>

<p>But even more important, control charts become vehicles for motivating process owners and process participants. They quantify and display process goals for all to see.</p>
<p>Indeed, both tools are necessary for almost every Lean Six Sigma project. One helps us through the design process and the other assures that our hard work will deliver results.</p>

<p>Kanban is a simple concept that can be very beneficial in process control. Kanban is a chart with columns representing the stages in a process and down each column in the chart the units sequenced through the process are moved from the first to last column. It is a visual display of movement through a process, also referred to as a <a href="https://en.wikipedia.org/wiki/Kanban_board" target="_blank" rel="noopener">Kanban Board</a> or Kanban Chart (shown below).</p>

<h2>Top 2 benefits of Kanban</h2>

<li>Kanban charts help us understand the current workflow from the beginning of the process through to the end.</li>
<li>Kanban charts are used to support a Pull system where the downstream steps pull work from<br />
upstream into the next available downstream step. For example, patients would be pulled from<br />
the waiting line to registration and then pulled into the next available examination room and so<br />
on throughout the entire system.</li>
</ol>
<h2>Understanding the benefits of Kanban</h2>
<p>Consider the implications for a four-star restaurant where every dish is made to order. A customer places an order. This initiates a process where the order is them pulled through the system: from food preparation to dinner table. In this way supply and demand are balanced.</p>
<p>The concept of Kanban is quite versatile, and it can be used in almost any industry. Further it focuses on the entire value chain from raw materials to the shipment of the final product or in the service sector from requests for service to service delivery.</p>
<p>Since Kanban manages the flow through a process, it is obvious that the application of this concept Kanban requires constant monitoring. But the benefit of constant monitoring is that it can be used to avoid bottlenecks and delays. For example, when a task is halted during one of the process steps a tag or marker can be placed on that item thereby bringing attention to the fact that whatever is interfering with moving the item through to the next step in the process requires attention.</p>
<h2>Where can Kanban be used?</h2>
<p>There are countless applications. In staffing it can be used to maintain control of staff who are available, those who have been assigned to tasks, and those who have completed their assignments. In operations it can be used to maintain control over the progress of inputs through a process, in recruiting it can be used to monitor job applicants as they proceed through a multi-step interview process, and in procurement it can be used to monitor the status of orders.</p>
<p>Because it is simple and so visual, Kanban has become a very useful tool in the Lean Six Sigma Toolkit.
